*The Cottleville Smiles Dental Membership Plan is not dental insurance, but a membership plan for prepaid services performed exclusively at Cottleville Smiles. If periodontal disease or gingivitis is present, additional treatment will be necessary prior to your cleaning. Membership Plan is billed annually; $50 reinstatement fee if plan lapses. We will auto renew your plan unless you notify us of cancellation. Annual rate may increase upon plan renewal without notice. You will not receive a membership card. Your plan’s effective date will be on file with our office. *20% off dental treatment does not include any cosmetic dentistry or implant supported dentures.
